Greater New York Nursing Services, a vibrant home healthcare agency, actively seeks a full time UAS Reviewer to ensure quality and accuracy on documentation and compliance with state program standards by carefully reviewing the assessments performed by UAS Field Assessors in accordance with agency policies. UAS REVIEWER DUTIES AND RESPONSIBILITIES + Reviews UAS and workbook documentation to ensure quality and accuracy. + Compares UAS and workbook to ensure correlation. + Serves as an expert clinical resource for the UAS Field Assessor Team. + Provides Field Assessors with feedback on their documentation as well as guide them towards documentation improvement + .Identifies and assists with Nurses continuing education and training of Agency policies and procedures for UAS and assessment. + Develops and fosters key relationships within the healthcare community to support the provision of services. ? Maintains a clinical focus with an underlying emphasis on operational excellence EDUCATION REQUIRED + ? Graduate of 2 - 4 year accredited Nursing program. + ? Bachelor?s Degree in Nursing or advanced degree is preferred. + ? Active and unrestricted NY State Registered Nurse (RN) License. + ? CCM Certification + ? Knowledge of current community health practices for the frail adult population, behavioral health and cognitively impaired seniors and disabled including knowledge of physical health, aging and loss, appropriate support services in the community, frequently used medications and their potential negative side-effects, depression, challenging behaviors, Alzheimer?s disease and other disease related dementias EXPERIENCE AND SKILLS REQUIRED: + Minimum of three to five years of demonstrated clinical and supervisory experience in a MLTC setting. + Proven knowledge of UAS-NY Assessments. + Knowledge of MLTC regulatory requirements. + Knowledge of current community health practices and resources available. + Solid assessment and documentation skills. + Ability to effectively communicate verbally and in written formats at all levels + High level of professionalism with ability to handle sensitive and confidential information. + Meticulous attention to detail and strong organizational skills.
