REGISTERED NURSE JOB DESCRIPTION REPORTS TO: Director of Patient Services DEPARTMENT: Nursing TIME / STATUS: Regular, Fulltime, Per Diem, Non-exempt BENEFITS: Full GENERAL DESCRIPTION:Under the general direction of the Director of Patient Services; assessing patients for start of service; supervising caregivers; employee health assessment DUTIES AND RESPONSIBILITIES:1. Providing nursing services including: pouring medications into pill boxes as needed and training Home Health Aides (HHAs) to assist in giving injections to patients under special circumstances as ordered by the medical doctor.2. Monitoring patient flow.3. Supervising Home Health Aides and patients as needed. 4. Writing out and updating Plans of Care for the patients.5. Maintaining updated clinical files of referrals and providing referral information to the Director of Patient Services.6. Ordering supplies for patients as needed.7. Participating in general and nursing staff meetings as required.8. Performing all duties and services in full compliance with HIPAA laws.9. Performing other related duties as may be assigned by the Director of Patient Services.10. Answering to the overall manager when needed.11. Scheduling and taking Home Health Aides through mandatory in-services.12. Maintaining Patient Rights as permitted by law.13. Checking aides' PPds and forwarding results to Mobile Health when needed.14. Performing other routine procedures (e.g. wound dressing) when necessary.15. Performing patient assessments.16. Opening new cases as assigned by the Director of Patient Services.17. Performing unannounced visits to patient homes when needed.18. Staying in constant communication with patient medical doctors.19. Requesting and obtaining doctors' orders (M11qs) for patients.20. Taking Home Health Aides orientation and through in-services when needed.*** Based on Company needs additional responsibilities may added on. QUALIFICATIONS: 1. Must be licensed and a currently registered professional nurse in New York State.2. Preferably two years experience in home care.3. Preferably minimum one-year supervisory experience.4. Must have a college degree in nursing.Preferable Skills:1. Being multi-lingual is a plus2. Must be able to relate appropriately to, communicate effectively with, and develop trusting relationships with diverse patient population and with staff.3.
